Yes. Post-hospital transitions are a large part of what we do. We can coordinate with the discharge planners and have medication reminders, meals and follow-up appointments in place for the first days home.

Usually not. We're set up for regular support on a set schedule with a consistent caregiver, with a minimum of four hours per visit, 16 to 20 hours a week. If you need occasional help, we'll tell you so on the first call.
